3. Appointment Requests

Submitting an appointment request through the Site is a request only and does not guarantee that an appointment will be scheduled or that a specific date, time, or provider will be available. We will contact you to confirm any scheduled appointment.

4. SMS Communications

If you opt in to receive text message appointment notifications, you consent to receive automated SMS messages from Rejuvenation Clinic at the mobile number you provide. These messages are limited to appointment-related notifications (e.g., confirmations, reminders, rescheduling) and are not used for marketing purposes.

  • Message frequency varies based on your appointment activity.
  • Message and data rates may apply.
  • You may opt out at any time by replying STOP to any message. Reply HELP for assistance.
  • Consent to receive SMS messages is not a condition of receiving services from Rejuvenation Clinic.
  • Carriers are not liable for delayed or undelivered messages.

Your mobile information and SMS consent will not be shared with third parties or affiliates for marketing or promotional purposes.
